Masks are mandatory during all shoots at Idle Eyes

We ask that everyone who comes to the studio please fill out one of these forms for AHS tracking purposes.

We also have a digital (forehead) thermometer at the studio, and your photographer may ask you to take a temperature reading before your shoot.

If you have a fever, cough, shortness of breath, difficulty breathing, sore throat, runny nose and/or any other flu-like symptoms, we ask that you do not enter our studio.

If you answered “YES” to either of the above questions, we kindly ask that you not enter the studio at this time as you are required to self-isolate as per AHS rules.
